I have reviewed the options above and approve my student’s choices. I am aware that Practicum assignments will be made based on these choices and that assignments will not be changed except for extreme medical need or family emergency. I an issue arises with financial hardship, an alternate local service opportunity will be assigned at the Practicum Committee’s discretion. I’ve read the policies in the Practicum Catalog and agree to abide by them. Overseas mission trip Practicum selections must be accompanied by the Short-Term Mission Application as well. This form is available at the front desk of the Jr/Sr High.
