Physical Form

A new sports physical is required for all student athletes each academic year and must be performed by a licensed healthcare professional.

• Participation Physical Evaluation: This form must be completed by parents/guardians and students AND a registered healthcare professional. This form is hosted on CHSAA’s website, as it is required by them (though the completed form must be turned in to FRCS).

Unless otherwise indicated, these forms are required for all families with student athletes. PLEASE NOTE: When filling out your student athlete’s name, please spell it EXACTLY how you did on the primary packet form above. This helps us track form completion. Thank you.
